Licensed Practical Nurse/LPN (Shreveport/Bossier Area) – We are seeking applicants for a Licensed Practical Nurse (LPN) position for our new Community Health Worker program. The Community Health Worker LPN will work with individuals and caregivers engaged with the health care system who need assistance navigating healthcare. The LPN will work closely with referral agencies, medical providers, primary care teams, and other agencies to improve the member’s care and outcomes. The LPN will serve as a liaison/connector between the member, care team, and the community. The LPN will help to bridge conversations with members and remove barriers that prevent them from accessing health and social services. Successful candidates will have a passion for and professional knowledge of underserved populations and their communities. Requirements include certification as a Licensed Practical Nurse; education above high school diploma or GED; a minimum of one year’s experience working in health/social services; ability to be sensitive and responsive to diversity, equity and inclusion; and ability to communicate effectively with clients, co-workers, volunteers, and supervisor. Candidate must have a valid Driver’s License with a good driving record; liability insurance that meets minimal requirements; vehicle must display current safety inspection sticker; acceptable criminal background check results; and must have a cell phone.

Residential Trainer (Shreveport) – We are seeking candidates for part time and full time Residential Trainer positions. Candidates must be available to work 12-hour shifts nights, weekends, and holidays. Residential Trainers ensure the safety and wellbeing of individuals who reside in a residential setting. Primary responsibilities include providing training to residents; transporting residents to appointments; planning and conducting structured activities; and coordinating with the resident’s Case Manager concerning resident’s progress, including appropriate change to the service plan. Requirements include a high school diploma or GED; prior work experience in a program that serves persons with intellectual disabilities preferred; CMA certification preferred; and current certification in CPR/First Aid. Hourly rate of pay is $12.00. Candidates must pass a background check and have a valid driver’s license.

Licensed Mental Health Professional – Office-based Assessor (Shreveport) – Primary responsibilities include maintaining scheduled intake appointments, conducting face-to-face interviews, completing a thorough biopsychosocial assessment for adults and youth, developing a personalized treatment plan, completing all other intake/consent forms thoroughly, accurately, and in a timely manner. Additional responsibilities would be providing psychotherapy, individual intervention, parent/family intervention and crisis intervention, and risk assessments. Minimum requirements include a master’s degree in counseling or a related field; and full licensure in the State of Louisiana.

Program Assistant/Driver Lighthouse (Shreveport area) – We have part time Program Assistant/Driver positions open at our Travis Street LightHouse after-school program. The Program Assistant/Driver utilizes an agency vehicle to transport program participants to/from the program site. Additionally, they work with the Program Coordinator to provide instructional and recreational activities for program participants five to thirteen years of age. Working hours are from 2:00 p.m. to approximately 7:00 p.m., Monday-Thursday. Candidates must pass a background check; have a valid Louisiana Chauffeur’s License; and a good driving record. Prior experience working with youth is preferred.

Mental Health Professional II (Desoto Parish) – We currently have an opening for a full time school-based Mental Health Professional in our Youth Behavioral Health Program. Primary responsibilities include providing supportive counseling, individual intervention, parent/family intervention and crisis intervention for school aged children with mental health and/or behavioral health issues. Additional duties include collecting data for the initial assessment; conducting face-to-face interviews with the recipient and their family, which shall include school and home visits; providing crises intervention support as needed; and ensuring all records are accurately maintained and all invoicing is submitted in a timely manner. Minimum requirements for a Mental Health Professional II include a master’s degree in counseling, psychology, or social work. Preference will be given to those fully or provisionally licensed in the State of Louisiana.

Direct Service Worker/Personal Care Attendant (Shreveport/Bossier area) – We are currently seeking individuals who are interested in working with intellectually disabled clients in a direct service worker position. Personal Care Attendants ensure the daily safety and wellbeing of developmentally disabled clients in a residential setting.
Responsibilities include ensuring that clients follow proper activities of daily living (dressing, personal hygiene, meal preparation, laundry skills, etc.). Additionally, Personal Care Attendants transport clients to medical appointments, grocery shopping, etc.
Requirements include a high school diploma or GED, a valid driver’s license, dependable transportation, and acceptable results of a criminal background check. Prior experience working with the disabled population is preferred.
